Tissue Viability Society Champion Programme

The Society of Tissue Viability are running a Skin Care Champions Programme. It’s been designed in response to the needs of the social care sector to support, develop and improve skin and wound care practice within care homes and home care agencies. Tissue Viability Service

The tissue viability service aims to provide specialist advice on non-healing and complex wounds, in addition to the provision of education and advice to health care professionals involved in wound management or in the prevention of pressure ulceration.  It is important that referrals to the service are appropriate and timely to ensure the best outcome for the patient in addition to making the best use of a limited resource.

The documents under Downloads are word documents, they are for the referral process.

We ask that you refer patients to the TV team using the appropriate referral form (see downloads) and email to our generic email address – tissueviabilityteam@mpft.nhs.uk. Please ensure that your contact email address and telephone number are within the referral, so we are able to discuss your referral with you. We will no longer action emails requesting advice without the appropriate documentation attached (e.g. Referral, photos etc.) and will send these back. Once a referral is received by the TV Team, a member of the team will contact you to discuss the patient further.
