14 years
- My parents/carers can consider if I need support from a social worker
- What might I need to learn to help me be more independent?
- Annual Review. In my Education Health and Care Plan (EHCP) Annual Review we will talk about my future goals and how I am going to get there. This includes my education, employment, living independently, my health and relationships.
16 years
- Do I need support to make my own decisions, or can I do this for myself?
- What will my day look like when I'm not at school or college?
- I have a right to have my wishes and feelings heard.
- My social worker will make sure I have an adult social worker if needed.
- Annual Review
- If I don't have a social worker I can ask for an assessment

18 years
- Am I entitled to any benefits?
- My adult social worker will plan with me the support they assess I need to start from my 18th birthday.
- My support plan will be reviewed with me and the important people in my life.
- Annual Review. At my EHCP annual review we will talk about whether I can progress further with my education or training and review progress towards outcomes.
25 years
