Completion and submission of this booking form confirms delegate agreement to Care Market Development Team terms and conditions, as follows:
- Course bookings can be made by accessing our website https://www.staffordshireconnects.info/cmdt and following the “Training and Events” link, or by contacting us on 0800 328 4122 or via cmdt@staffordshire.gov.uk.

Places will be allocated to delegates on a “first come, first served” basis, with confirmation of places sent to delegates via the email address provided at the time of booking.

Initial confirmation generated by the website that a booking form has been received should not be considered final confirmation of a place at any event. Final confirmation of places at any course/event will be provided by email from CMDT.

Places at any one course/event will normally be limited to two per organisation.

It is an expectation that all delegates have discussed their learning requirements and intended attendance at a course or event with their manager ahead of booking, and gained management consent to book a place. Submission of the booking form shall be considered to confirm this consent.

Registration information, including directions to the venue and timings, will be sent to the delegate using the email address provided at registration prior to the event. CMDT will not be responsible for the non-arrival of registration information. Any delegate not receiving the registration information by 24 hours prior to the event should contact us on 0800 328 4122.

CMDT will share the names of delegates attending a particular course with the third party course/event provider delivering the course, to facilitate on-the-day course registration. By booking a place at a course, delegates consent to CMDT sharing their names with third party course/event providers.

All materials provided for CMDT courses/events including graphics, code, text products, software, audio, music and design are owned by CMDT or CMDT’s third party course/event providers. No content in whole or in part of these materials may be copied, reproduced, uploaded, posted, displayed, linked to or used in any way without the prior written permission of CMDT and/or third party course/event providers.

CMDT aims to ensure that its courses/events are accessible to all. If you have any specific needs, such as dietary or access requirements, please advise us as soon as possible, preferably at the time of course booking, and we will contact you to discuss your requirements. It is the responsibility of delegates to inform CMDT about any specific needs or requirements they may have.

A minimum of 10 working days’ notice of cancellation is required should a delegate become unable to attend a course or event. Notice should be provided via email to cmdt@staffordshire.gov.uk wherever possible. If the delegate does not have email access, telephone cancellation may be provided by contacting CMDT on 0800 328 4122.

Delegates are able to nominate a substitute to take their place at a course/event, up until the day of the event. Where a substitute delegate attends a course/event, no cancellation charges will be levied.

It may be necessary for CMDT to change the content and timing of a course; the trainer; the date or the venue. In the unlikely event of the course being cancelled by CMDT, a full refund will be made unless the delegate transfers their booking to an alternative date for the course.

For all bookings, the liability of CMDT shall be limited to the amount of the fee paid to CDMT by the delegate or their employing organisation. CMDT will not accept liability for any additional costs incurred by delegates.

CMDT will, on occasion, contact individuals registered with our website to share information about upcoming events, training courses or other sector news/updates. You can opt out of receiving this information at any time by emailing “OPT OUT” to cmdt@staffordshire.gov.uk
Addendum (Applicable from 01.12.2020 in relation to changes pertaining to virtual training course delivery and introduction of an online payment system)
- Upfront and online card payments now apply to secure attendance on all charged CMDT courses/events.
- Delegates or their managers booking onto courses/events which are subject to attendance fees will receive an immediate automated email providing instructions on how to pay online when a course booking has been made.
- Course bookings will remain provisional (place(s) booked on course(s) will be held) until online payment has been made. They will remain provisional for a period of up to 2 working days from the point at which a provisional course booking was made. Failure to pay during this time period will result in the course booking being cancelled and the place being released. Email notifications will be sent when cancellation of a provisional course booking has been made.
- In relation to substitutions in point 11 of the above terms and conditions, delegates or their managers are able up until 8 a.m. on the morning of the course/event to nominate a substitute. This nomination must include the full name and the contact email address of the nominated substitute and be sent to the CMDT@staffordshire.gov.uk mailbox. Details of substitutes sent after this timepoint will not be considered and reimbursement of course fees will not be possible.
- Delegates or their managers who cancel a confirmed place or notify CMDT by email of the cancellation less than 10 working days from the virtual course/event taking place, will be subject to payment of the full course fee with no reimbursement.
- Delegates who do not attend on the day of the booked course/event and who fail to organise a substitute (as in addendum 4 above) will be subject to payment of the full course fee with no reimbursement.
- In the event that minimal attendance has not been met to cover course costs within one month prior to course/event taking place, it may be necessary for CMDT to change the date of the course. In this event, a full refund will be made unless the delegate transfers their booking to an alternative date for the course.
