This document is for notification of a permanent exclusion only.  Please download from the action column to the right of the page.

This form must be completed at the time of the permanent exclusion.

Send it to inclusion@staffordshire.gov.uk as soon as possible along with a copy of the letter sent to parents.

This is to ensure that the sixth day alternative educational provision is put into place by the local authority.
