Care Navigation Service - South Staffordshire District

The Care Navigation Service has been developed to support older people to keep healthy, safe and independent in their own home.

Care Navigation brings all of the professionals together, in one place, and with access to the right information, to ensure that vulnerable older people get the right care, at the right place, at the right time.

The aim of the service is to work alongside local health care, social care, and third sector teams and provide 'supported signposting' for local, frail, older people, some with long term health conditions and enable them to live more independent, healthier lives.

This is a free service which supports older people who are undergoing change in their lives and may need information and help accessing services.

This should enable the most appropriate care and support is provided by the right practitioner at the right time.

Who is eligible?

People aged 50+ who have either multiple long term conditions, risk of or experiencing social isolation and loneliness, mild to moderate mental health conditions, experiencing difficulties from a change in social circumstances or a carer who is struggling.

People must be registered with one of the following practices to access the service: Wombourne Gravel Hill medical practice, Wombourne Dale medical practice, Kinver Moss Grove medical practice and Claverley medical practice (including Pattingham branch surgery), Russell House medical Practice, Codsall, Bilbrook Medical Centre, Lakeside Medical Centre Perton, Tamar Medical Centre, Perton and Featherstone medical practice.

Who can refer?

Referrals are made directly to the Care Navigator programme by Community Matrons through GP Practices, MDTs, District and Community Nurses and other health, social care and community Professionals.

How to refer

Care Navigators will aim to contact patients within 48 hours of receipt of referral and will aim to complete a guided conversation within 10 working days of the initial contact.
