This aim of the service is to support children and young people attending special schools in their school environment in order that they can play a full and active role in school life, remain healthy and achieve their academic potential.

Who can use the service?

Parents, guardians, carers, professionals and young people up to age 25 years in special schools.

How can I refer?

You need to contact the appropriate teams via school or email

What happens after referral?

School nurse will contact parents and young person to discuss any issues and further referrals to be made if appropriate.

Who will we see?

Your referral will be allocated to a member of the team with the appropriate skills to deal with the concern.

What happens when I come to the service?

We will discuss your concern and either resolve the issue ourselves or refer to appropriate service.

What happens when I become an adult?

If appropriate transition will be discussed and plans put in place.
