Community Children's Nursing Service
A team of experienced children's nurses who support children and young people who have either short term or long term healthcare needs at home and in community settings.
Nurses work in partnership with children, young people and their families and carers to assess healthcare needs and provide nursing care, support, teaching and advice to enable care to be managed safely at home as far as possible.
Who can use your service?
Children 0-18 years who have a nursing need that is not met by any other service.
Are there any costs?
This is a NHS Commissioned service. There is no cost for service users.
How can I refer?
Referrals can be made by GP, Advanced Nurse Practitioner or hospital.

What happens after referral?
The Children’s Community Nurse will contact the family directly and agree a plan of care.
Who will we see?
You will see a Children Community Nurse.
What happens when I come to the service?
The Children’s Community Nurse will see you in your home or community setting. This will be agreed with you and your family. The nurse will assess what care you need and agree a plan of care with you and your family. The care will be different depending on what your needs are.
What happens when I become an adult?
The Children’s Community Nurse will access your nursing needs before you are 18 years old . If you are likely to need continuation of care after 18 years the Children’s Community Nurse will agree a plan with you so that your care needs can be met in adult services. You and your family will be involved in this process.
Where similar services are not available alternative provision will be sought.
