Lead by a fully accredited health coach who will work with you to establish a healthy lifestyle plan to help improve overall health and wellbeing.

Health coach has a specialism in supporting people living with Dementia and Diabetes.

Service details

  • Puts you at the centre of your improved health journey
  • Your coach partners, with you as a guide, will create your personal roadmap to whole health and wellness
  • You will learn to cultivate and sustain lifestyle and behaviour change to help you manage existing conditions
  • You'll learn to create the health you've always wanted
  • Health and wellbeing plan established by you
  • Access to a fully trained health coach
  • Weekly support from a volunteer mentor to help keep you on track
  • Regular phone contact
  • A range of healthy lifestyle services you may access
  • Low carb advice
  • Weekly exercise program
  • Peer to peer support

Health mentors

  • Our volunteers and staff benefit from full DBS checks and training
  • Volunteer mentors for this role have access to specific training courses relating to dementia, Parkinson's and diabetes
  • We will actively encourage service steering for those with lived experience of the conditions supported
  • Our volunteer mentors could be college / university students on adult social care courses
  • Our volunteers do not replace healthcare professionals nor provide care. They are however happy to signpost to these services
  • Transport by volunteer to services in the community can be facilitated
  • The service is aimed at early intervention and will support those newly diagnosed or with early onset of conditions
  • All services and delivery will be guided and overseen by a fully trained health coach and experienced member of the MHA team.
