We provide Community Children’s Nursing services to children and young people residing in North Staffordshire and Stoke-on-Trent with varying health needs ranging from acute to complex and life limiting/ life threatening.

  • Hospital @ Home 0-16
  • Palliative & Complex Care Team 0-19

Are there any costs?

There are no costs. This is a commissioned NHS service-free at point of need.

How can I refer?

Referral’s ‘are only accepted from either GPs, Advance Nurse Practitioners or a consultant 

What happens after referral?

  • Hospital @ Home - We will home visit or telephone and support acute assessment and treatment in the child and family’s own home, dependant on acute illness and need. We provide support and health education to parents and carers in order to facilitate self-care and independence.

  • Palliative and Complex Care Team -You will be assessed to identify which element of the service you require - complex or life limiting. You will be assigned a named nurse whom will assess and support your nursing need. We will individualise your care planning & support you towards self-efficacy.

Who will we see? 

You will be offered support from various members of staff within the service, senior community nurses, community nurses, psychology support & family support staff- all dependant on diagnosis and need.

What happens when I come to the service?

You will be contacted to inform you of the referral to gain your consent- you will be assessed as to identify your suitability for any element of the service based on clinical need.

  • Palliative and Complex Care Team-You will be assessed to identify which element of the service - complex or life limiting. You will be assigned a named nurse whom will assess and support your nursing need. We will individualise your care planning & support you towards self-efficacy.

What happens when I become an adult?

We offer an individual transition care plan between paediatric and adult health services, we will support & coordinated a smooth seamless transition into adulthood, and ensuring adequate time is given to access appropriate training and procurement of equipment and engagement of adult services.
