Important information for parents during COVID-19 lockdown

We consist of multi-disciplinary teams providing both community and inpatient care. Many of these services are provided in collaboration with partner agencies, both statutory and non-statutory.

Our services are organised into a series of pathways;

  • Non-psychosis team
  • Psychosis team
  • Intensive lifeskills team
  • Memory service and dementia team

The pathway teams are supported as required by the Access Team and Crisis Team. For urgent responses within four hours to people in crisis.

Our vision is to work with service users, carers and key stakeholders to develop a range of services and support networks that ensure that individuals are empowered to live as optimum a life as possible. We believe services should be as close to home as possible and indeed in the majority of cases people will be able to be supported to remain within their home environment.

Who can use the service?

  • Anybody aged 16 years or older who is worried about their mental health.
  • Anybody aged 16 years or older using South Staffordshire and Shropshire Healthcare’s adult mental health services who needs to contact us out of hours.
  • Anybody aged 16 years or older who cares for someone receiving adult mental health services from us.
  • Anybody aged 16 years or older concerned about the mental health of an adult family member or close friend.
  • Professionals needing advice and guidance regarding mental health issues.

How do I refer?

What happens after referral?

Once you are referred you will be assessed by one of our team, the team has qualified staff to speak to, who are knowledgeable about different services and options. They will listen to your concerns and give you support and ideas about what might be helpful. We will help you to decide what the next steps will be and, if necessary, offer you a suitable appointment.

Who will we see?

You will initially be seen by a qualified member staff member.

What happens when I come to the service?

We will carry out an assessment of your needs. This is an opportunity for you to ask questions and for us to find out how our services can help you towards your recovery.

You will be encouraged and supported to get involved in the planning of your treatment. Where necessary, we can also provide access to specialist services such as sign language to help you participate fully. 

Care and support planning, along with the assessment, is part of a process known as care co-ordination. There will always be a member of staff identified to lead your care. This person may be referred to as: your care co-ordinator, key worker or lead professional.

Once your needs have been assessed, we will work with you to draw up your personalised plan of care. This may be in the form of a ‘Care and Support Plan’ or a letter describing your care. You should be given a copy, so if you’re not, please ask.
