We support patients over the age of 18 post discharge from hospital who require, a combination of nursing and therapy interventions, to remain in their own homes and avoid unnecessary hosptial admissions.  This can be with or without care support. We aim to enable them to recover fully and meet their full rehabilitation potential by maximizing independence.   

Who can use our service?

Anyone over the age of 18 that is referred in via the hospital or by any community health care professional.

Are there any costs?

There are no costs.

How can I refer?

You can refer via the Home First referral line.  Referrals have to be made by a GP or community Health care professional who have seen the patient and taken recent observations.  The referral line 0300 123 0983.

What happens after referral?

The referral is passed to the appropriate team covering that area.  A decision is made regarding who is the most appropriate clinician to visit. Further assessments and interventions are planned from this initial contact.

Who will I see?

It could be an Occupational Therapist, Physiotherapist, Registered General Nurse or Advanced Nurse Practitioner.

What happens when I come to the service?

You will receive all appropriate assessments from within the team, who will liaise with the GP etc as required to provide holistic care in the home environment.
