About me

I qualified as Foot Health Practitioner (FHP) in 2013. Training at the College of Foot Health Practitioners achieving a Level 4 Diploma in Foot Health Practice. FHP’s are private sector providers of regular and routine foot care. FHP’s are not Chiropodists or Podiatrists and do not come under HCPC regulation. However I am a member of the ‘Alliance of Private Sector Practitioners’ which provides professional advancement, Continuing Professional Development, and listing on the Professional Standards Authority 'Accredited Register of Foot Health Practitioners'.

Appointments

No commitment is required. You may book your next visit in advance or on a ‘ring up’ basis. There is no recommended interval for treatment, most people find 6 – 8 weeks about right.

What to expect

No preparation by the client is required, all equipment is provided. Foot washing is not offered but feet are cleansed before treatment. A Visual assessment is made and treatment can consist of nail cutting and filing, removal of any callous and corns with pads/dressings and cream applied all as appropriate. 

Fees / Payments

The fee is per person and is fully inclusive. Payment by Bank transfer to 08 93 00   87146518 is preferred or payment by debit card – a card reader is available. Cash or cheques accepted if no alternative.

Cancellations / missed visits

You will not be charged for cancellations or missed appointments. As much notice as possible is appreciated as cancellations do impact on the schedule and driving route. Repeated short notice cancellations or missed appointments will result in the service being withdrawn.  I may sometimes ask to cancel or re-schedule bookings your cooperation is appreciated

You may be aware that new regulations came into force in May 2018 regarding Data Protection. This means that ‘Sarah Lee’, has a duty to inform you of any personal information that she holds about you, why this information is taken, who has access to the information and how it is stored and used securely. This document has been compiled to explain this.

What Data will be collected/processed?

Your name, address, telephone number, email address, next of kin details, date of birth, GP details, medical details and treatment records. 

For what purpose is the Data collected and used?

To enable ‘Sarah Lee’ to be able to appropriately provide treatment to you.  If appropriate/necessary, to contact your GP with regard to any findings that could need onward referral if outside of our remit of treatment.  We may also use your contact details to provide you with appointment reminders as necessary via telephone, SMS or email. 

Who will have access to the Data?

Sarah Lee solely has access to all data held. At no point will any of your information be passed to a third party. 

How will the Data be stored?

All information given is held in paper form and kept in locked storage.  [Patient telephone number and address are also be stored on Sarah Lee’s mobile telephone phone which is PIN protected)

How long will the Data be stored?

In line with the legal requirements of holding medical data, your record is stored for 7 years from the date of last treatment by us.

Please note: that your right to be forgotten cannot override the legal requirement to keep medical notes for the mandatory period

When will the Data be destroyed?

All data will be destroyed after 7 years, or in the case of a minor once that minor reaches the age of 25 years
