SEND Keyworker Contacts
We are responsible for the assessment of children and young people aged 0-25 with complex special educational needs who may need an education, health and care plan.
The service writes and maintains education, health and care plans and is responsible for reviewing plans on at least an annual basis.
The service is also responsible for determining levels of high need provision and specialist placement.
