First Person Complex Care

We provide tailored care packages to support children and young adults with complex health and social care needs to live safely and comfortably in their own homes with their families, to attend school or college and to access activities which are meaningful and enjoyable for them in the community.
We work closely with families, Clinical Commissioning Groups, Local Authorities and Case Managers to deliver flexible, responsive, high quality, home/community-based care. We also deliver expert advice and bespoke training solutions for individuals and organisations working with children and young adults who have complex needs.
Our services are provided across Staffordshire and the surrounding areas.

Our care services will be delivered using a case management model to ensure a highly personalised and bespoke approach for each client and their family.
We are able to deliver fully managed care packages where we work with the family/funder to develop and deliver a fully managed, tailored care package from a few hours a week to 24 hours a day. We will work closely with the family to recruit, employ, train and manage nursing and care staff and we will charge an inclusive hourly rate for this service using a transparent, straightforward fee structure.
We can also work with Personal Health Budget/Direct Payment recipients to establish their care package. In this scenario, First Person Complex Care will offer a ‘menu’ of services to provide as much or as little support as the client and their family requires in order to establish and maintain an effective and competent care package.
