*Staffordshire Family Support and Outreach Service


Who is this service for?
This service is for families with children aged 0-19 years and up to 25 years for those with Special Educational Needs and Disabilities (SEND) who need support, so they feel empowered to manage challenges and take greater control over their own family’s health and wellbeing.
This is a local service, available free of charge to all families resident in Staffordshire and where appropriate, to those with children registered at a Staffordshire school.
How to access this service?
We welcome self-referrals and referrals from any professional. Referrals can only be made with the agreement of the family.
How we work, together with you
We work with the whole family, delivering our support in the family home and other community settings, like schools or Family Hubs.
A named Family Support Worker will be in contact with you to make sure this is the right type of service for you and your family.
For the Family Support Worker to understand your support needs fully, they complete an assessment with you, your family and other relevant individuals you identify together. This will build on existing strengths and avenues of support.
We recognise a range of factors impact on families reaching out for support. For this reason, we can support you with:
- Parenting skills and strategies
- Child's personal, social and emotional development
- School readiness
- Building healthy relationships
- Improved family relationships
- Staying safe in the community
- Meeting children's needs impacted by cost-of-living pressures
- Access to local services and facilities
