Learning disability specialist nurses

We support adults with a diagnosed learning disability to access primary care (doctors, dentists, podiatry, etc). We do this by working with health staff to help them understand the needs of people with a learning disability, and how they can help people better.  We also work with people with learning disabilities and complex needs that might need extra support and co-ordination to meet their health needs.

Who can use your service?

Anyone who is aged 18+ and diagnosed with learning disabilities. People are usually on their GP’s learning disability register. 

Are there any costs?

There are no costs.

How can I refer?

Anyone can refer. We have a nurse covering each area of Staffordshire:  See "Who to contact" section above.

What happens after referral?

Someone will be in contact with you to arrange an appointment if appropriate.

Who will we see?

You will see the nurse that covers your area (see "Who to contact" section below)

What happens when I come to the service?

The nurse will visit you at home or somewhere that is convenient to you.  You will be asked questions about your health and to explain any concerns you have.  Actions will be discussed and agreed.
